Inspector Notes

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the code compliance inspection. Subsidy standards that are not covered in code were reviewed during this SHSI.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

Standard 8VAC20-790-540-B
The children?s records are required to contain required information. The records for Child #1, Child #2, Child #3, Child #4, Child #5, Child #6 and Child #7 did not contain the address for each custodial parents? place of employment and did not contain the address of at least one person designated by the parent to contact in case of an emergency and the parent cannot be reached. The record for Child #7 did not contain proof of a child?s identity and age as stated in ? 22.1-289.049 of the Code of Virginia.
Plan of Correction: We are creating new forms. The new forms will include requesting the appropriate information.
Standard 8VAC20-790-640-A
Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. Cleaning chemicals were unlocked sitting on top of cabinets in eight classrooms.
Plan of Correction: We will purchase equipment to ensure cleaning chemical are safely secure and contained.

Medication, except for those prescriptions designated otherwise by written physicians? order, including refrigerated medication and staffs? personal medication, shall be kept in a locked place using a safe locking method that prevents access by children. Prescription medications were located in unlocked classroom bags in five classrooms.
Plan of Correction: We will be ordering secure medication pouches that lock for each child that has medication present at the center.
