Inspector Notes
This monitoring inspection was initiated by licensing staff and concluded on 06/15/2022, between the hours of 10 am to 1 pm. There were 55 children present, ranging from infant to preschool-age, with 12 staff supervising across 6 classrooms. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition. A total of 5 children records, 2 board officers records, and 5 staff records were reviewed.
Information gathered during the inspection determined that there were (2) two non-compliances with applicable standards. These violations were documented on the Violation Notice issued to the program.

1. On the date of inspection (06/15/2022), the TB documentation on file for Staff #2 was dated - 12/01/2021.
2. The start date for Staff #2 was listed as - 05/23/2022.
Evidence -
1. On the date of inspection (06/15/2022), deteriorating wood was observed on the center's playground; specifically on the right side of the "fort" structure with the blue tarp. The wood appears to be heavily weathered with a raised surface.
2. Children were observed playing on this piece of equipment on the date of inspection.
