Inspector Notes
An unannounced monitoring inspection was conducted today from 10:30am-12:30pm. There were 63 children directly supervised by 7 staff. The physical plant, 5 staff records, 7 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in transitioning outside and playing outside. Diaper changes and handwashing were also observed. There was an assortment of age appropriate materials for the children in care. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Staff 1 (working as a volunteer) was observed working in ration on the date of inspection. Their record did not contain documentation of a national criminal record check.
Evidence:
Staff 1 (a volunteer), was observed working with children in ratio on the date of inspection. Their record did not contain a sworn statement or affirmation document.
Evidence:
Staff 1 (a volunteer) was observed working in ratio on the date of inspection. Their file did not contain the results of a Virginia central registry check.
Evidence:
1. Child 4's most recent immunizations were documented at 17 months of age.
2. Child 5's most recent immunizations were documented at 12 months of age.
Evidence:
1. Two cabinets were observed containing spray disinfectant. The keys to the cabinets were observed to be stored in the lock.
2. A chemical sponge was observed sitting on an open sink ledge.
Evidence:
On the date of inspection written procedures for prevention of shaken baby syndrome or abusive head trauma could not be produced. The center management told the inspector that they are currently working on developing them
Evidence:
The allergy lists were observed posted in plain view throughout the center.
Evidence:
1. Child 8's medication authorization does not contain the signature of a physician.
2. Child 9's medication authorization does not contain the signature of a physician and of the child's parent.
3. Child 10 and Child 11's authorization does not give "Your Child's Place" permission to administer medication. The authorization form lists that Fairfax County Public schools may administer the medication.
Evidence:
Child 9's medication was present in the center on the date of inspection. The medication authorization, signed buy the child's parents expired on 1/24/2023.
Evidence:
Seven incident/accident reports were reviewed. Five reports did not contain documentation of actions taken to prevent further occurrences. Five reports did not document the time that parents were notified. One did not contain the date the parent was notified.
