Inspector Notes

An unannounced code compliance inspection was conducted on January 5, 2024.There were 9 children present, ranging in ages from 3 years to 4 years years, with 3 staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision and procedures. A total of 3 children and 3 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

The provider must send documentation to the LI that the background checks have been requested no later than 10 days following this notification of the Acknowledgement Form. Once the background check is received by the center, the results must be forwarded to the licensing inspector.

Based on a review of staff records, 2 staff do not have a search of the central registry within 30 days of employment.
Evidence: Staff #1 and #2, both employed 7/2023 do not have documentation of the search of central registry.
Plan of Correction: Both staff acknowledge that the search has been completed. Richmond was contacted this date and the search was completed in August but did not reach the providers email. The provider is working with Richmond to receive the results.
