Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and a virtual tour of the program.

A monitoring inspection was initiated on 1/10/2022 and concluded on 1/12/2022. The director was contacted by email to initiate the inspection. There were 27 children present and 5 staff. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 5 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and interview it was determined the center did not obtain a copy of the results of a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.
Evidence:
1. The record for staff #3(hired 11/16/21) indicated they had lived in North Carolina within the preceding five years. There was no child abuse and neglect registry search for North Carolina in the record.
2. The record for staff #5(hired 8/2/21) indicated they had lived in Louisiana within the preceding five years. There was no child abuse and neglect registry search for Louisiana in the record.
3. Staff #2 confirmed that these documents were not available during inspection.
Plan of Correction: Center will request the child abuse and neglect registry search for staff #3 and #4.
