Inspector Notes

An unannounced monitoring inspection was conducted today from 8:45am-10:20am. There were 11 children (40 points) directly supervised by the provider and 2 assistant caregivers. The home, 3 caregiver/household member records, 3 children?s records, evacuation drills, and emergency supplies were inspected. Children were observed playing freely. Areas of non-compliance are identified in the Violation Notice.

Based on record review, 1 of 3 children's records did not contain all required information.

Evidence:
1. The record for Child #3 did not contain the phone number of a second required designated persons to contact in case of an emergency if the parent cannot be reached
Plan of Correction: The provider will talk with the parents to obtain the information.
Standard 8VAC20-800-780-B
Based on record review, the provider did not review with the parent the emergency contact information required to ensure the information is correct, and obtain the parent's signed acknowledgment of the review annually.

Evidence:
1. The most recent review of the emergency contact information signed by parents in the record for Child #2 was dated 8/1/2022.
2. The most recent review of the emergency contact information signed by parents in the record for Child #3 was dated 8/3/2022.
Plan of Correction: The provider will review the information with the parents today.

Based on record review, the family home did not obtain documentation of additional immunizations for one child under the age of 2 years.

Evidence:
1. The most recent documentation of immunizations in the record for Child #2 (age 15 months) were dated 09/25/2022.
Plan of Correction: The provider contacted the parent to request updated documentation of immunizations during inspection.
