Inspector Notes
A renewal inspection was conducted on January 17, 2023 between the hours of 3:25 pm and 4:50pm. There were 4 children present, with one (1) caregiver supervising within the supervision guidelines. Children were observed sleeping, diapering, handwashing, eating snack and playing with age appropriate toys. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. There was no medication on sight. A total of 4 child records, caregiver and household member records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

3Evidence: The slide and ladder on the playground equipment is approximately 2 feet from the wooden border that surrounds the equipment and is not at least 6 feet from the perimeter of the wooden border.

Evidence:
1.The provider did not clean and sanitize the diapering surface after each use.
2. Child #2's diaper was changed, but the diapering surface was not cleaned and sanitized afterwards.
Evidence:
1. Child #2 (start date 6/3/22) did not have verification of identity and age in their record.
2. The Provider confirmed verification of identity and age was not obtained.
