Inspector Notes

Licensing Inspector Anderson conducted an unannounced Monitoring inspection in response to a self report of an incident that occurred at the center on April 30, 2024. Licensing Inspector initiated the investigation on May 1, 2024 and conducted the onsite inspection on May 15, 2024. Findings were reviewed with the center on July 15, 2024. The Licensing Inspector reviewed 1 child record and 1 staff record, along with any requested program records submitted by the facility to determine if required documentation was complete.

Violations have been cited related to information submitted by the Center. Please see Violation Notice for more details.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on
July 20, 2024. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-A
Based on interviews, written statements and video, the Center failed to ensure staff who are supervising children, always ensure their care, protection, and guidance.

EVIDENCE:
On Tuesday, April 30, 2024 at approximately 12:15 pm, Staff #1 was escorting the 3 year old classroom outside to the playground. According to the video, Staff #1 and the children stopped at the corner of the gymnasium building when Staff #1 grabbed Child #1 by the wrist, picked them up off of their feet and carried them approximately 10-15 feet around the corner of the building because of their misbehavior.
Plan of Correction: Staff resigned. The Code of Conduct on 4/13/2024. Staff had a challenging behavior training on 5/28/2024 held by United Way. Staff will continue child abuse training yearly.

Based on interviews, written statements and video, the Center failed to ensure forbidden actions are not used as a means to discipline the children in care.

EVIDENCE:
On Tuesday, April 30, 2024 at approximately 12:15 pm, Staff #1 roughly handled Child #1. Video revealed that Child #1 was grabbed by the wrist, lifted off of their feet and carried approximately 10-15 feet around the corner of the building because of their misbehavior.
Plan of Correction: Staff have been reminded of VDOE Licensing Standards & will be reminded frequently through the year. Staff have been reminded about forbidden actions and re-signed code of conduct.
