Inspector Notes

An unannounced monitoring inspection was conducted September 14 2022 between 9:00am and 10:00am. There were 37 children in the direct care of 4 staff members. Children were observed working on individual stations, washing hands, having snack, and participating in group work. A review of staffing records, children's records, the physical space of the center, the playground, emergency drills, and attendance was conducted. Citations are listed on the violation notice. Based on record review, the center did not ensure that all employees undergo a background check every five years after their initial background check.

Evidence: Staff 2 record contained a central registry search check dated February 8 2017.
Plan of Correction: This will be resolved today.

Based on observation, the center did not keep hazardous substances in a locked place using a safe locking method that prevents access by children

Evidence: Cleaning wipes were observed not in a locked location.
