Inspector Notes
An unannounced renewal inspection was conducted on 11/03/2022 from approximately 12:30 p.m. ? 3:45 p.m. Caregiver, household member, children?s records and the physical plant were reviewed. There were 7 children present with 2 caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies, and nutrition. A total of 5 child records were reviewed. Caregiver and household member records were reviewed. The information gathered during the inspection determined violations with applicable standards.

5Evidence:
The physical exam dated 06/02/2021 for C1, enrolled 10/31/2022 was conducted more than 12 months prior to attendance.
The physical exam dated 12/06/2019 for C2, enrolled 10/31/2022 was conducted more than 12 months prior to attendance.
Based on observation the provider failed to ensure that stationary outdoor playground equipment shall be firmly anchored with ground supports.
Evidence:
The child swing set used by children was not anchored in the ground. The licensing inspector was able to lift the swing set legs off the ground.
Evidence:
Upon arrival at to home, children were observed to be resting on mats without linens.
Evidence:
Provider failed to ensure that the 911 emergency phone number was posted near the home telephone.
Evidence:
The emergency drill documentation dated June 2022 ? October 2022 did not document: the method used for notification of the drill; number of caregivers participating; number of children participating; any special conditions simulated; the time that it took to complete the drill; problems encountered, if any; and weather conditions.
