Inspector Notes

A subsidy health and safety inspection (SHSI) was conducted on 5/23/2024 from approximately 1:23 p.m. ? 4:45 p.m. Caregiver, household member, children?s records, and the physical plant were reviewed. There were 6 children present with 1 caregiver supervising initially and later 2 caregivers. Five child records were reviewed, two caregiver records, and one household member record was reviewed. A licensing inspection was also conducted on this date. Both inspections combined cover all subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. The information gathered during the inspection determined a violation with applicable standards or laws.
