Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on March 16, 2021 and concluded on April 1, 2021. The director was contacted by telephone to initiate the inspection. The center is currently closed due to a health pandemic. There were 22 children present and 4 staff on their last operating day, March 13, 2020. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 3 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Inspection was amended 4-13-2021.

Standard 22VAC40-185-160-C
Based on record review and staff interviews, the license did not ensure at least every two years from the date of the first initial screening or testing a staff member shall obtain and submit the results of a follow-up tuberculosis screening.

Evidence: The record for staff #2 did not include an updated TB screening/test. The most recent one available for review was dated 2-28-2017.
Plan of Correction: Trying to get appointments for TB screening. Will have it done ASAP.
Standard 22VAC40-185-260-A
Based on review of documentation and staff interviews, the center did not obtain an annually updated fire inspection.

Evidence: The center director confirmed an updated fire inspection had not been obtained. The most recent inspection is dated 12-17-2018.
Plan of Correction: Appointment for fire inspection to be completed.
Standard 22VAC40-191-40-D-1-A
Based on record review and staff interviews, the license did not ensure a new person designated as applicant or agent who is or will be involved in the day-to-day operations of the facility has completed the sworn statement or affirmation and a search of central registry (CPS) whenever an applicant or registrant changes before the end of 30 days after the change.

Evidence: The center director confirmed Agent #1 and Agent #2 did not obtain a CPS check or have a sworn statement. The center director confirmed Agent #1 and Agent #2 have been on the board since 2020.
Plan of Correction: CPS has been sent off.

Based on record review and staff interviews, the license did not ensure a new person designated as applicant or agent who is or will be involved in the day-to-day operations of the facility has obtained a finger print check by the end of the 30th day of beginning service.

Evidence: The center director confirmed Agent #1 and Agent #2 did not obtain a fingerprint check. The center director confirmed Agent #1 and Agent #2 have been on the board since 2020.
Plan of Correction: I am working on getting an appointment for agent #1 -agent #2 for fingerprinting.
