Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated on January 27, 2022 and concluded on January 31, 2022. The director was contacted by telephone and a virtual inspection was conducted. There were 11 children present, ranging in ages from 3 to 4, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and provider interviews, the provider did not ensure an employee of a licensed child day center had obtained fingerprinting /criminal records check prior to the 1st day of service.

Evidence: The record for staff #1 and staff #2 who were both working during the inspection did not have documentation of obtaining a fingerprint check prior to employment.
Plan of Correction: Finger print appointments have been made for both staff.
Standard 8VAC20-780-60-A-8
Based on record review and staff interviews, a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Evidence: The record for child #2 did not include allergy action plans for diagnosed food allergies listed are (raw peaches and raw tomatoes).
Plan of Correction: Once the parent gets the child's allergy action plan from their doctor. We will add it to their chart.
