Inspector Notes
A monitoring inspection was initiated and concluded on April 11, 2023. There were 15 children present, ranging in ages from 3-5 with 4 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records, 5 staff records and the background checks for other persons were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22.1-289.035-B-2
Based on record review and staff interviews, the licensee did not ensure, all staff obtained a Criminal Records Check (CRC) prior to the first day of employment.
Evidence: The record for Staff #2 (date of hire 4-4-2023) did not include a CRC. Staff #2 confirmed the missing information and that they were working on the day of the inspection.
Plan of Correction: Staff #2 CRC was received 4-12-23
Standard 22.1-289.035-B-4
Based on record review and staff interviews, the licensee did not ensure, all staff had obtained an out of state criminal history name check (CRC) prior to the first day of employment and an out of state child abuse and neglect search (CPS) request by the 30th day of employment.
Evidence: The record for Staff #5 (date of hire 3-21-2022) did not include the CRC and a CPS for the state of Arizona where they had lived within the last 5 years. Staff #2 confirmed the missing information and that Staff #5 was working on the day of the inspection.
Plan of Correction: Staff #5 will receive a certified letter form Arizona stating the state does not share CRC/ CPS
Standard 8VAC20-780-160-A
Based on record review and staff interview the licensee did not ensure documentation of the Tuberculosis screening shall be submitted at the time of employment and prior to coming into contact with children.
Evidence: The following staff did not have TB screenings as required.
1)Staff #2 did not have documentation of a TB screening conducted and the first date of employment was 4-4-2023.
2)Staff #5?s first date of employment was 3-21-2022 and TB screening was conducted 4-7-2022.
Plan of Correction: Staff #2 will complete TB screening as required. Staff #2 will come up with tracking system to ensure staff TB occur prior to come contact with children.
Standard 8VAC20-780-240-A
Based on record review and staff interviews, the licensee did not ensure each staff completed the Virginia Department of Education-sponsored orientation course within 90 calendar days of employment.
Evidence: The record for Staff #3 (date of hire 3-21-2022) and Staff #5 (date of hire 11-2-2022) did not include documentation of completing the required orientation.
Plan of Correction: Orientation will be concluded for staff #3 & #5
Standard 8VAC20-780-240-C
Based on record review and staff interviews, the licensee did not ensure each staff member had completed orientation training in all required facility specific topics prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities.
Evidence: The record for Staff #4 (hire date 11-2-22) did not include information indicating orientation training had been completed to include to prevention of abusive head trauma and safe sleeping practices.
Plan of Correction: Staff #4 no longer work for the organization as of 4-14-23. orientation will be conducted first day staff is hired.
Standard 8VAC20-780-40-K
Based on review of center documentation and staff interviews, the licensee did not ensure the center shall develop written procedures for prevention of shaken baby syndrome or abusive head trauma, including coping with crying babies, safe sleeping practices, and sudden infant death syndrome awareness.
Evidence: Center Director confirmed the center does not have written procedures for safe sleep.
Plan of Correction: Staff #2 will share & create a procedure for safe sleep. Also will inform the board members regarding policy.
