During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site focused inspection was initiated on 7/14/2025 and completed on 7/21/2025, as a part of an Intensive Plan of Correction (IPOC). The on-site inspection began at 11:40 am and ended at 1:00 pm. The inspector reviewed compliance in the areas listed above. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 7/30/2025. A POC submitted after this date will not appear on the public website.

Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for Child #1, enrolled for a month, was missing the city, state, and zip for one emergency contact. The record for Child #3, enrolled for a month, was missing the city, state, and zip for both emergency contacts. The record for Child #4, enrolled for a month, was missing names of persons authorized to pick up the child and programs or schools that the child is concurrently attending. The record for Child #5, enrolled for a month, was missing the second parents name address and phone number, names of persons authorized to pick up the child, programs or schools that the child is concurrently attending and an allergy care plan for a diagnosed food allergy.
Plan of Correction: Child #1 review all emergency contact form and corrected all missing or incomplete information for each enrolled child on 7/15/2025. Child #4 now has the authorized to pick up at program listed. Child #5 file information was updated and fixed on 7/15/2025. Child allergy care plan will be in file by August 1 and the parent was notified on 7/14/25.
