The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Each report shall include dates immunizations were received and shall be signed by a physician, his designee, or an official of a local health department. The record for Child #4, immunization record was not signed by a physician.

Each child shall have a physical examination by or under the direction of a physician: Before the child's attendance; or within 30 days after the first day of attendance. Child #4, in attendance for more than two months, did not have documentation of a physical examination.

Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment. Staff #1 TB was dated five months prior to date of employment, Staff #4 TB was dated 14 months prior to date of employment, and Staff #5 TB was dated eight months prior to date of employment.

Staff shall complete annually a minimum of 16 hours of training appropriate to the age of children in care. Staff #2 had 8.5 hours out of the 16 required hours of training in 2024.

(Repeat) Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for Child #1, was missing the work phone number and place of employment for both parents. The record for Child #2, was missing the work phone number for one parent.
