Inspector Notes
A monitoring inspection was conducted today, June 27, 2023. The inspector was on site from approximately 10:30 am until 2:30 pm. The director was present and assisted in the inspection. There were 143 children present, ranging in ages from six months to twelve years, with 30 staff supervising. Children were observed participating in a variety of activities including center play, working with clay, water table, story time and group games. School age alternated between theater, art, board games and time at the pool. Classrooms Interactions between staff and children were positive. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of ten child records and thirteen staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations
2Evidence:
1. The national fingerprint check for Staff #10, employed 07/05/2022, was dated 07/12/202
exceeding the date of employment.
2. The national fingerprint check for staff #12, employed 06/14/2023, was dated 02/27/2023, exceeding 90 days prior to employment.
3. National fingerprint background checks are required to be obtained prior to employment and cannot precede the employment date by more than 90 days.
Evidence:
1. The record of staff #1, employed 04/10/2023, did not contained the results of a central registry report exceeding 30 days from employment. The director stated a follow-up to the status of the central registry was conducted however the original request was made on 05/03/2023 exceeding seven days from employment.
2. The record of staff #10, employed 07/05/2022, contained a central registry check dated 09/27/2022 exceeding 30 days from the date of employment.
