Inspector Notes

A monitoring inspection was conducted today, January 11, 2024. The inspector was on site from approximately 10:00 am until 2:30 pm. The director was present and assisted in the inspection. There were 117 children present, ranging in ages from three months to six years, with 26 staff supervising. Children were observed in a variety of activities including story time, sensory, finger paints, puzzles, magnetic blocks, and center time. Children had science class and time outside. Interactions between staff and children were positive. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and
nutrition. A total of ten child records and ten staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-270-A
Based on observation, the center did not ensure all areas of the center, inside and outside, were maintained in a clean, safe, and operable condition.

Evidence:

A light switch cover in the bathroom in the Avenues, was cracked and missing a corner piece leaving a sharp edge and exposure to the interior of the light switch.
Plan of Correction: Per the director: I will have the light switch cover replaced by tomorrow and request staff let me know when equipment is broken.

Based on observation, the center did not ensure when a child was placed in an infant seat, the protective belt was fastened securely.

Evidence:

On January 11, 2023, the licensing agents observed one infant sitting in infant seats at
the feeding table. The infant did not have the protective belt fastened. Staff sitting at the feeding table with the infant acknowledged the belt was not fastened.
Plan of Correction: Staff fastened the safety belt immediately after the unbuckled belt was pointed out. Per the director: Staff will fasten the safety belt immediately after placing an infant in the seat.
