During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 09/17/2025 and completed on 09/17/2025. The on-site inspection began at 1:17pm and ended at 3:49pm. The inspector reviewed compliance in the areas listed above. There were 53 children present and six staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 09/25/2025. A POC submitted after this date will not appear on the public website.

Standard 22.1-289.035-B-3
The center must request a search of the central registry prior to the employee's first day of employment.

Staff #1, who had been employed for five months, had a central registry search requested 10 days after the first day of employment
Plan of Correction: Although the employee was having trouble receiving the emails sent to her, we will not be starting employment till all is completed.
Standard 8VAC20-780-160-A
The center is required to obtain documentation of a negative tuberculosis (TB) screening at the time of employment and prior to coming into contact with children.

Staff #2 is currently working and had a TB screening that was dated one day after their first day of employment.
Plan of Correction: Staff will not be able to start till all paperwork is completed.
Standard 8VAC20-780-160-C
A tuberculosis (TB) screening is required every two years from the last screening.

Staff #3 updated screening was due one month ago. The updated screening has not yet been completed.
Plan of Correction: Updated screening has been done. Going forward, screening must be done before continuing work.
Standard 8VAC20-780-240-A
Staff are to complete the VDOE sponsored online orientation course within 90 days of hire.

Staff #1, who is working with children, has not yet completed this course which is overdue by two months.
Plan of Correction: Staff was not allowed to return to work till class was completed.
Standard 8VAC20-780-80-B
The center shall inform the department's representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention.

A child sustained an injury that required outside medical attention on 08/05/2025. The center did not notify the department.
Plan of Correction: Going forward we will report and follow up on any injury that leads to outside medical care.
