Inspector Notes
An unannounced, on-site focused inspection was initiated and completed on 2/13/2025, as a part of probation. The Department issued a Notice of Intent to place the licensee on probation and require the licensee to contact parents, guardians, or other responsible persons regarding health and safety violations on 10/23/2024 as a result of high-risk violations
The on-site inspection began at 11:00 am and ended at 11:30 am. The inspector reviewed compliance in the areas listed above. There were 56 children present and 14 staff. The Staff and children?s records were not reviewed during the focused inspection.
This inspection included tour of the facility, interviews, observations.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
