During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 3/28/2025 and completed on 4/9/2025 as a part of probation. The Department issued a Notice of Intent to place the licensee on probation and require the licensee to contact parents, guardians, or other responsible person regarding health and safety violations on 10/23/2024 as a result of high-risk violations
The on-site inspection began at 1:15 pm and ended at 4:20 pm. The inspector reviewed compliance in the areas listed above. There were 55 children present and 21staff. The inspector reviewed 6 children?s records on site on 3/28/2025 and 10 staff records electronically on 3/31/2025.
This inspection included document review, tour of the facility, interviews, observations, and measurements.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. Staff #7, did not have documentation of requesting an Abuse and Neglect Registry check and a Criminal record check from one state where staff #7 resided within the past five years. These out of state background checks are overdue by more than 7 months.
Plan of Correction: This was an unintended error. Out of state background checks were initiated for staff #7 but sent to the wrong state.
We have requested the correct state's criminal check using link provided.
We are working with the employee to get needed paperwork signed so we can request the out of state CPS today.
To prevent an oversight like this happening again, effective today, we put a 2-step file verification process in place. This will require two people to review a recruit's file to ensure all proper background checks are completed.
Standard 8VAC20-780-280-B
(Repeat Violation) Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. A spray bottle of disinfectant was located in an unlocked cabinet under the diaper changing table and a can of spray disinfectant, a bottle of glue and residue remover and a bottle of floor cleaning concentrate were located in an unlocked cabinet under the adult handwashing sink. These items were each labeled ?Keep out of reach of children" and were at floor level, within reach of children, in a classroom where six children, ages 23 to 27 months old were napping.
Plan of Correction: Staff stated these items had been used when they cleaned up after the children settled on their cots for rest period and had inadvertently not been locked yet. Staff immediately locked the cabinets at the time that these items were observed and pointed out to them.
