During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 3/28/2025 and completed on 4/9/2025 as a part of probation. The Department issued a Notice of Intent to place the licensee on probation and require the licensee to contact parents, guardians, or other responsible person regarding health and safety violations on 10/23/2024 as a result of high-risk violations
The on-site inspection began at 1:15 pm and ended at 4:20 pm. The inspector reviewed compliance in the areas listed above. There were 55 children present and 21staff. The inspector reviewed 6 children?s records on site on 3/28/2025 and 10 staff records electronically on 3/31/2025.
This inspection included document review, tour of the facility, interviews, observations, and measurements.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

We are working with the employee to get needed paperwork signed so we can request the out of state CPS today.
To prevent an oversight like this happening again, effective today, we put a 2-step file verification process in place. This will require two people to review a recruit's file to ensure all proper background checks are completed.
