During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced, on-site focused inspection was initiated and completed on 7/7/2025, as a part of probation. The Department issued a Notice of Intent to place the licensee on probation and require the licensee to contact parents, guardians, or other responsible persons regarding health and safety violations on 10/23/2024 as a result of high-risk violations.
The on-site inspection began at 12:18 pm and ended at 12:56 pm. The inspector reviewed compliance in the areas listed above. There were 74 children present and 26 staff. Staff and children?s records were not reviewed during the focused inspection.
This inspection included tour of the facility, interviews, and observations.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
