Inspector Notes

This monitoring inspection was initiated by licensing staff and concluded on 09/23/2022, between the hours of 10 am to 12:30 pm. There were 63 preschool children present with 9 staff supervising across 9 separate classrooms. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication; special care and emergencies; and nutrition. A total of 10 children records, and 5 staff records were reviewed.

Information gathered during the inspection determined that there were (2) non-compliances with the applicable standards, code, and/or law. These violations were documented on the Violation Notice issued to the center.

Standard 22.1-289.035-B-4
Based on review, 1 of 5 staff files indicated that the child day center did not obtain a copy of the results of a criminal history record information (CRC) check, and a search of the child abuse and neglect registry (CPS) or equivalent registry from any state in which the individual has resided in the preceding five years before reporting to the child day center.

Evidence -
1. On the date of inspection (09/23/2022), the sworn statement (dated - 06/29/2022) completed by Staff #2 indicated that they lived in another state or US Territory within the past 5 years. Neither a CPS check or a CRC from this location was available for review.
2. A Virginia fingerprint check was on file. Therefore, an additional Sex Offender Registry check was not required.
3. Staff #2 was observed on-site on the date of inspection providing care. The start date for Staff #2 was listed as 09/06/2022.
Plan of Correction: Will obtain and contact this location for results.
Standard 8VAC20-780-160-A
Based on review, 2 of 5 staff files indicated that not all were able to obtain tuberculosis (TB) documentation that was completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.

Evidence -
1. On the date of inspection (09/23/2022), the following staff had on file TB documentation that was outside the 30 day window from their individual start dates: Staff #1 (start date - 09/06/2022) TB documentation dated - 07/14/2022, and Staff #2 (start date - 09/06/2022) TB documentation dated - 06/29/2022.
2. This observation was confirmed on the date of inspection by the Center Director.
Plan of Correction: Will follow and comply by have all new staff submit TB documentation prior to start date while staying within the 30 day window. A revision of the offer letter may also indicate this requirement.
