The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.

Staff #1, employed for two months, indicated on their sworn statement that they had lived out of state in three states over the past five years. Staff #1s record did not have documentation of requesting a central registry check and sex offender registry check from three states and did not have documentation of a criminal history record check from two states.
Plan of Correction: Employee lived in 3 states for religious mission. Their last day of employment is 12/19. Even though they are leaving in a week, we have started all background check procedures for all states.
