Inspector Notes

Today an unannounced Monitoring Inspection took place between the hours of 12:10pm and 1:10pm. There were 39 children and 10 staff, within the supervision guidelines. The children were observed reading books, coloring, cleaning, putting together puzzles and other organized activities. A complete inspection of the physical plant, children and staff records, evacuation drill log, and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Based on a review of children's records, 1 out of 3 children did not have a physical examination prior to attendance or within the time period required.

Evidence:
1. Child #2 (doe: 8/29/2022) did not have documentation of a physical examination on file during the time of the inspection.
Plan of Correction: We will ask the parents for the physical examination today.

Based on observation, food brought from home was not labeled with the date for the day.

Evidence:
1. There were 2 children's lunch boxes observed that were labeled with their name, but no date during the time of the inspection.
Plan of Correction: We will make sure all children's food from home is labeled with their name and date each day.
