Inspector Notes

An unannounced inspection was conducted on 10/30/2024 in response to a complaint received by the licensing office on 10/28/2024 relating to an allegation in the area of staff to child ratios. This inspection included a tour of the facility, interview and observations.

The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance(s) with applicable code sections, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/7/2024. A POC submitted after this date will not appear on the public website.

Violations

1
Standard 22.1-289.031-A-3-b
The child day center shall employ supervisory personnel according to the following ratio of staff to children: One staff member to five children from ages 16 months to 24 months.

In the toddler room, (age 16-24 months) one staff was one to two children over ratio several times during a two week period. The duration of time was approximately 5 minutes or less during each occurrence.
Plan of Correction: We had a staff member leave, and ratio was fixed as soon as the position was filled with proper background checks completed. We are hiring substitute teachers from within the church congregation and trying to make room in the
budget for floaters who can also substitute when we have unexpected vacancies.
