Inspector Notes
A subsidy health and safety (SHSI) inspection were initiated on 11/28/23 and concluded on 11/28/23. There were nine children present with 2 staff supervising ranging in age from 13 months to four years. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined non-compliance with subsidy regulations and violations were documented on the violation notice issued to the facility.
Violations

1. Child 1 has food allergies per the child's record. An inspection of the center was completed and a written list of food allergies, sensitivities and dietary restrictions was not found.
2. The provider stated they did not have a written list of children with food allergies, sensitivities and dietary restrictions.
Evidence:
1. The record for child 1, start date 10/30/23, has a note from the physician documenting allergies as egg whites, peanuts and soybeans. There is no allergy care plan in the record.
2. The provider verified they do not have an allergy care plan for child 1.
