Inspector Notes

A subsidy health and safety (SHSI) inspection were initiated on 11/28/23 and concluded on 11/28/23. There were nine children present with 2 staff supervising ranging in age from 13 months to four years. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined non-compliance with subsidy regulations and violations were documented on the violation notice issued to the facility.

Standard 8VAC20-790-170-G
Based on record review and interview, the provider failed to ensure a current written list of all children's allergies, sensitivities and dietary restrictions were accessible to caregivers that was dated and kept confidential.

Evidence:

1. Child 1 has food allergies per the child's record. An inspection of the center was completed and a written list of food allergies, sensitivities and dietary restrictions was not found.
2. The provider stated they did not have a written list of children with food allergies, sensitivities and dietary restrictions.
Plan of Correction: A list of the one child with allergies was made and posted during the inspection. There are no other food related allergies, sensitives or dietary restrictions. The provider will maintain the list in the future.

Based on record review and interview, the provider failed to obtain a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Evidence:

1. The record for child 1, start date 10/30/23, has a note from the physician documenting allergies as egg whites, peanuts and soybeans. There is no allergy care plan in the record.
2. The provider verified they do not have an allergy care plan for child 1.
Plan of Correction: The parent of child 1 was called during the inspection to advise an allergy care plan is needed. In the future any new child that enrolls with a food allergy will be required to provide an allergy care plan prior to attendance.
