Inspector Notes

A complaint inspection was initiated and concluded on May 17, 2022. A complaint was received by the department regarding allegations in the areas of supervision. The licensing inspector was onsite May 17, 2022 from approximately 10:00am to 11:00am. Observation of the religious exempt child day center were made, documentation was reviewed and interviews were conducted.

The evidence gathered during the investigation did not support the allegation of non-compliance with standards or law. Although not related to the complaint, a violation was discovered and a violation was documented on the violation notice issued to the program.

Based on observation and interviews, the child day center did not employ supervisory personnel according to the required ratio of staff to children.

Evidence:
Upon arrival at the center on 05/17/2022 at approximately 10:00am, Staff #1 was supervising 14 children (three to five years of age). The licensing inspector observed Staff #1 was the only staff in the building with the 14 children. Staff #2 arrived at the center at approximately 10:15am.
Plan of Correction: I was at the center this morning. When I left the center, Staff #1 was in ratio. There were no more than nine children. Additional children arrived at the center after I left to run an errand.
