Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated on 4/14/21 and concluded on 4/20/21. The assistant director was contacted by telephone to initiate the inspection. There were 22 children present and 5 staff. The inspector emailed the assistant director a list of items required to complete the inspection. The Inspector reviewed 2 children's records and 2 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Based on interview and record review, the center did not ensure that each child shall have a physical examination by or under the direction of a physician before the child's attendance or within one month after attendance.

Evidence:
1. The record for child 1 (date of enrollment: 11/2/20) does not contain documentation of a physical examination.
2. The assistant director confirmed that the record for child 1 is lacking documentation of a physical examination.
Plan of Correction: The center responded with the following: The center will contact the child's caretaker today to request a copy of the child's most recent physical.
