Inspector Notes

A renewal inspection was conducted on 04/14/2023. There were 19 children present, ranging in ages from infant to school age, with 7 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 5 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program and discussed with the program director during the exit interview.

Based upon review of children's records and staff interview, the facility has not ensured that each child has a physical examination by or under the direction of a physician before attending or within 30 days after attending.
Evidence:
1. The record provided for child 4, who began attending on 8/30/2021, did not include the results of a physical examination.
2. Staff 1 verified that the record for child 4 did not contain the results of a physical examination.

Standard 8VAC20-780-280-D
Based upon observation and staff interview, the facility has not ensured that sanitizing materials are not located above food, food equipment, utensils or single-service articles and are stored in areas physically separate from food.
Evidence:
1. There was a container of aerosol sanitizer on a shelf in the infant room where infant food and food service items were stored.
2. Staff 1 acknowledged the sanitizer stored with food service items.

Based upon observation and staff interview, the facility has not ensured that in each grouping of children, at least one staff member who meets the qualifications of a program leader or program director is regularly present and that program leaders supervise no more than two aides.
Evidence:
1. Staff 1, who is program director qualified, stated that she oversees staff 2, 4, and 5, the assistants on duty in the infant toddler groupings.
2. There was no program leader qualified staff overseeing the preschool grouping. Staff 1 stated that they are hoping to get staff 6 program leader qualified.

Based upon observation and staff interview, the facility has not ensured that diaper changing stations are equipped with a diaper disposal system that is not hand operated.
Evidence:
1. There was no diaper disposal can for use by the diaper changing station in the preschool classroom.
2. Staff 1 stated that diapers are bagged and taken outside, not placed in a disposal can.
