The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. The record for staff 1 (employed for over 3 months) did not have documentation of requesting a criminal history check or a search of the child abuse and neglect registry from one state. The record for staff 2 (employed for over 4 months) did not have documentation of requesting a search of the child abuse and neglect registry from either state they have lived in within the past 5 years. The record for staff 3 (employed for a year and a half) did not have documentation of requesting a criminal history check from either state they have lived in within the past 5 years.
Plan of Correction: The center responded with the following: There has been a change in center management and some documents from staff records have not been able to be located. New out-of-state background check requests have been sent for staff 1, staff 2 and staff 3. Once received, the results will be forwarded to the licensing insepctor and placed in each staff's record.

Disposable diapers shall be disposed of in a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. The diaper disposal containers used by the center do not prevent the soiled diaper from touching the exterior surface of the of the storage system during disposal.
Plan of Correction: The center responded with the following: New diaper pails have been ordered for the center.

The center shall implement a monthly practice evacuation drill. A practice evacuation drill was not completed for the month of August 2025.
Plan of Correction: The center responded with the following: There was a change in center management during this time and the practice drill for August was overlooked. Going forward, evacuation drills will be practiced every month and documented on the practice drill log.

Each child's record shall contain all of the required information. The records for child 1 (enrolled for 6 months) and child 2 (enrolled for over 2 years) does not contain the addresses for the two emergency contacts.
Plan of Correction: The center responded with the following: Addresses for the emergency contacts for child 1 and child 2 will be obtained from the parents and documented in each child's record.
