During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced complaint investigation and inspection was initiated on 10/17/25 and completed on 11/5/25 in response to a complaint received by the licensing office on 10/17/25 relating to concerns in the areas of supervision and behavioral guidance. This inspection included document review, a tour of the facility, interviews and observations. The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to staff records and staffing and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 12/8/25. A POC submitted after this date will not appear on the public website.

Each grouping of children shall have at least one staff regularly present who meets lead teacher qualifications. Staff 1, who is not lead teacher qualified, was the only staff working in the preschool classroom from 10/15/25 through 10/17/25.
Plan of Correction: The center responded with the following: Staff scheduling procedures have been revised to prevent coverage gaps.

Documentation of at least 2 references to character, reputation, and competency are to be checked prior to employment. Staff 1, who has been working at the center for 2 months, did not have any completed reference verifications on file.
