Inspector Notes

An unannounced, on-site code compliance inspection was initiated and completed on September 15, 2025. The on-site inspection began at 10:00am and ended at 11:20am. The inspector reviewed compliance in the areas listed above. There were 27 children present with eight staff. The inspector reviewed six children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance(s) with applicable code sections, and violations are documented on the violation notice issued to the program.

The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.
The center did not request a search of the child abuse and neglect registry and sex offender registry request from one state for Staff #5, who has been employed for 26 days. Staff #5 was not present at the time of the inspection.
Plan of Correction: A sex offender request was completed at the time of the inspection and no results were found. An out of state child abuse and neglect search request was submitted on September 19, 2025. The facility will submit results to the licensing inspector when they return.
