Inspector Notes
An unannounced, on-site code compliance inspection was initiated on 9-23-2024 and completed on 9-26-2024. The on-site inspection began at 11:30am and ended at 1:25am. The inspector reviewed compliance in the areas listed above. The Department requested that the provider complete an Intensive plan of Correction (IPOC) as result of the last inspection related to Background Checks. The inspector reviewed 11 staff records on-site and 9-25-2026. This inspection included document review, tour of the facility and interviews.
Information gathered during the inspection determined non-compliance(s) with applicable code sections, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10-3-2024. A POC submitted after this date will not appear on the public website.
Violations
2The center is required to obtain a completed sworn statement prior to the employee's first day of employment. The sworn statement for Staff 2, 9 and 11, who are currently working, did not have signed statements and have been working for over a year.
The center must request a search of the central registry prior to the employee's first day of employment. Staff 5 has been employed for over a year and did not have a central registry search requested.
