Inspector Notes

A monitoring inspection was conducted on 4/14/22 from 4:40 pm to 5:45 pm. There were 12 children present with 3 staff supervising. The children were observed playing on the outdoor playground and getting picked up by their parents. Children were accounted for as they were leaving by completing the attendance. Compliance in the areas of administration, physical plant, staffing and supervision, medication, and emergencies were reviewed. A total of 2 children's records and 3 staff records were reviewed.

Based on records review, the center did not obtain a written allergy action plan with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction for each child.
Evidence:
1. Child # 1's record indicated that he had a diagnosed allergy to eggs.
2. The center did not obtain a written allergy action plan for Child # 1.
Plan of Correction: Parent has chosen to change from diagnosed food allergy to intolerance to food.
