Inspector Notes
An unannounced monitoring inspection was initiated on November 8, 2023 and concluded on November 22, 2023 in response to a self-report received from the facility regarding supervision. Licensing Inspector conducted classroom and playground observations, interviewed staff and reviewed facility record keeping. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation of video footage, interviews with staff and review of recordkeeping, facility did not ensure children under ten years of age are always within actual sight and sound supervision of staff.
Evidence: On October 26, 2023, between approximately 4:35 pm ? 4:45 pm, two staff were present with eight children (toddler and early preschool age). The group was transitioning from the rear playground to the front playground when they entered the building to walk up the hallway. Staff #1 was near the front of the line of children and staff #2 was at the rear of the line of children. Staff #2 stepped into a classroom immediately off the hallway to retrieve a child that ran into that room. While staff #2 was in that space, child #1 (twenty-one months old) walked into a separate adjoining classroom. Staff #1 and staff #2 continued with seven children to the front playground. Child #1 was found by parent alone in the hallway. Child #2 was without sight and sound supervision by staff for two minutes.
