Inspector Notes

An unannounced monitoring inspection was conducted on 1/24/2023. There were 28 children, ages 16 months -4 years, 4 staff and 1 administrative staff present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of disease, emergencies and nutrition. A total of 4 children?s records and 3 staff records were reviewed. The children were observed during active time on the playground, center time and story time. The inspector arrived for the inspection at 10:40am and departed the center at 1:11pm.

Discussion on standards related to special feeding needs.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on observation and staff interview, the center failed to ensure that staff-to-children ratio specified in Table 2 are required whenever children are in care.

Evidence: Administrative Staff #1 stated upon licensing staff?s arrival that a staff person in the 16-24 month classroom was out sick. The 16-24 month classroom had 7 children and 1 staff upon licensing staff entering the room for monitoring at approximately 10:50am and remained out of ratio at licensing staff?s departure from the center at 1:11pm.
Plan of Correction: Provider will ensure staff to children ratios are met.

Based on observation and staff interview, the center failed to ensure that staff-to-children ratio specified in Table 2 are required whenever children are in care.

Evidence: Administrative Staff #1 stated upon licensing staff?s arrival that a staff person in the 4?s classroom was out sick. The 4?s classroom had 12 children and 1 staff upon licensing staff entering the room for monitoring at approximately 11:00am and remained out of ratio at licensing staff?s departure from the center at 1:11pm.
