Inspector Notes
An unannounced Renewal inspection was conducted today. A total of 102 children with 19 staff were onsite. The children were transitioning from the lunch room to either nap time or quiet activities. School aged children were observed on the playground. Staff were strategically positioned on the playground to ensure appropriate supervision. Observation of all occupied classrooms was made. Staff interviews were conducted regarding emergency procedures and children's individual needs. Medication were stored in the classrooms along with a list of children's allergies. The classrooms were neat and maintained. Staff were observed sitting between children, gently patting backs to encourage sleep. In the infant classrooms, staff were observed sitting on the floor interacting with the infants with toys. In the second infant room, the children were observed sleeping in cribs with two staff rocking children to sleep. Ratios were maintained and exceeded the standard requirement. A total of 5 staff records and records/documentation were reviewed for 9 children which included medication authorizations and allergy action plans. Some violations were found today. A corrective action was discussed along with a date for corrections. This inspection occurred between 11:30am and 3:40pm. Thank you for your cooperation today. The allergy action plan for Child #2 was not updated. The most recent allergy action plan on file is dated August 2022.
Evidence:
1. The diaper surface in Class #10 had a crack in the plastic and is no longer non-absorbent.
Evidence:
1. The center's medication policy requires methods to prevent expired medication.
2. A liquid medication for Child #1 was found in Room 9 had expired in June 2023.
3. The medication policy requires duration of medication authorizations.
4. The medication authorization for Child #2and #4 did not include duration for the medication.
5. Child #1 has 2 long term medications. The authorizations for both medication was not signed by the physician.
