Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 05/08/2025, as a part of the licensure period. The on-site inspection began at 10a.m. and ended at 12:40p.m. The inspector reviewed compliance in the areas listed above. There were 121 children present and 34 staff. The inspector reviewed 10 children?s records and 7 staff records on-site. This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
3Staff #2, who had been employed for one month, did not have a central registry search requested.
A cleaning agent was stored on the same shelf and directly next to infant formula in one classroom.
Two topical ointments in one classroom had expired over 1 month ago.
