Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 9-25-2024 and completed on 10-2-2024. The on-site inspection began at 11:00am and ended at 12:00pm. The inspector reviewed compliance in the areas listed above. There were 36 children present and 4 staff. The inspector reviewed 5 children?s records and 4 staff records ectronically on 10-2-2024. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10-9-2024. A POC submitted after this date will not appear on the public website

The center is required to updated at least annually the injury prevention procedures based on documentation of injuries and a review of the activities and services. There was not an updated injury plan.
Plan of Correction: The Education Manager ensured that each classroom has an updated injury prevention plan in the classrooms. The Education Manager made a copy for the classroom on 09/27/2024 and handed it to the site supervisor.

The center is required to keep a current list of all known allergies, sensitivities, and dietary restrictions in each room or area where children are present. The list must be dated and kept confidential. The list in the room 2 did not include the physician?s documented allergy for child #4.
Plan of Correction: The Health Coordinator will add Child #4 documented allergy to the list on 10/08/2024.
