Inspector Notes

An unannounced non-mandated focused inspection was conducted on May 15, 2023 between the hours of 9:50am and 10:35am. There was one (1) class observed playing on the playground with a total of 17 children with two (2) staff members supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, emergency procedures, and child and staff records. The information gathered during the inspection determined non-compliance with applicable standards or law. Based on record review and interview, the center failed to inform the department's representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention.
Evidence:
1. Director emailed the department's representative on 05/12/2023 regarding the injury.
2. Injury report dated 3/31/23 was received on 05/12/23.
3. Director acknowledged the injury occurred on 3/31/23.
Plan of Correction: I did not realize I had to report the injury. I will report any injuries in the future.
