During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on September 3, 2025. The on-site inspection began at 9:55am and ended at 12:38pm. The inspector reviewed compliance in the areas listed above. There were 71 children present and 18 staff. The inspector reviewed seven children?s records and four staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. The information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-A
The center is required to have employees undergo a background check every five years. The most recent central registry check for staff #3 was overdue by five months and the most recent sworn statement was overdue by six months.
Plan of Correction: The staff completed a new sworn statement during the inspection. If the CRS is not located today, we will resubmit this week for a new one.
Standard 8VAC20-780-130-E
The center shall obtain documentation of additional immunizations once every six months for children under the age of two years. The last documentation of immunizations for Child #7, who is currently attending, was over a year ago.
Plan of Correction: We will contact the parent today that we need the updated immunization by 9/15/25.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for Child #1, enrolled for over two months, was missing the following required component: parent's work place. The record for Child #5, enrolled for over one month, was missing proof of identity as required.
Plan of Correction: We will contact the parents and get the required information by 9/5/25.
