Inspector Notes

Licensing Inspector conducted an unannounced Religiously Exempt Code Monitor Inspection on September 18, 2023. There were 68 children ages 2 1/2 to 5 years old present under the direct care of 13 staff. The Licensing Inspector reviewed 5 children?s records and 6 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on documentation review, the Center failed to establish and implement procedures for daily health screening and exclusion of sick children by a person trained to perform DHO screenings.

EVIDENCE:
There was not at least one person present who has received Daily Health Observation training of children.
Plan of Correction: Center Director will establish and implement DHO procedures and staff will complete the Daily Health Observations of children training. Documentation of the training will be maintained in each staff record.
