Inspector Notes

A renewal inspection was conducted on 05/22/2023 from 2 pm until 3:50 pm. There were 47 children present, ranging in ages from three months to five years, with 7 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and
supervision, programming, medication, special care and emergencies and nutrition. A total of nine child records, eight staff records, and seven medication records were reviewed.

Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on the review of 13 staff records, the program failed to ensure that every applicant for licensure undergoes a background check every five years.

Evidence:
1. The date of the last criminal background check in the record for Staff #6 was dated 05/23/2018.
2. The administrator confirmed that the repeat check had not yet been completed, but that an appointment was scheduled.
