Inspector Notes
An unannounced monitoring inspection was conducted today between the hours of 10:45 am-12:45 pm. There were a total of 52 children with 11 supervising staff members present. Children were observed during handwashing, singing, painting, engaged in group activities and outdoor play time. A selection of staff and children records, medications and other documents were reviewed. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Violations
6Evidence:
1. Staff #1(Start date-06/27/2022) lives in Washington DC. An out of state child abuse and neglect search request was not conducted for Washington DC until 08/25/2022.
2. Staff #3 confirmed that an out of state child abuse and neglect search request had not been conducted by the 30th day of employment.
Evidence:
1. Child #5's (Start date-01/07/2019) record did not contain documentation of additional immunizations. The last documented immunization was dated in December 2018.
Evidence:
1. Staff #1's (Start date-06/27/2022) record did not contain documentation of a complete orientation training of First Aid and CPR.
2. Staff #3 confirmed that Staff #1 did not complete First Aid and CPR orientation training.
Evidence:
1. In the Toddler classroom, one plastic edge protector located in the home living area is cracked and broken causing sharp edges. A second plastic edge protector located near the exit door is also cracked and broken causing sharp edges.
2. In the Pre-K classroom, one foot pedal on the trashcan is broken and not operational, and another foot pedal on the trashcan is missing and not operational.
3. In the Preschool classroom, the foot pedal of the trashcan is broken, a rug located in the library area is buckling causing a tripping hazard, and the bookshelves have warped plywood.
4. In the Early Preschool classroom, the plywood on the bookshelves are warped and cracked.
5. In the two's classroom, the foot pedal of the trashcan is missing and not operational.
6. On the playground, two tricycles have plastic seats that are cracked causing sharp edges.
Evidence:
1. In the two's classroom, the cots were not labeled for individual use by children.
Evidence:
1. Documentation of monthly evacuation drills were not maintained for the month of June and July.
2. Staff #3 confirmed that the evacuation drills were conducted for the months of June and July, but were not documented.
