Inspector Notes
An unannounced monitoring inspection was conducted today between the hours of 9:15am-11:30am. There were a total of 63 children with 11 supervising staff members present. Children were observed during diapering, circle time, singing,engaged in group activities and outdoor play time. A selection of staff and children records, medications and other documents were reviewed. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Violations
8Evidence:
1. Staff #2's record (Date of hire-10/11/2022) did not contain documentation of a tuberculosis screening dated within the last 30 days of the date of employment. The tuberculosis screening was dated 07/20/2022.
Evidence:
1. Staff #1's record (Date of hire-10/12/2022) did not contain documentation of a completed Virginia Department of Education sponsored orientation course within 90 days of employment.

Evidence:
1. In the Two's classroom, two electrical outlets were not covered with protective covers.
2. In the Pre-K classroom, two electrical outlets were not covered with protective covers.
3. In the Toddler classroom, two electrical outlets were not covered with protective covers.
Evidence:
1. Child #6's has albuterol on site. The albuterol was maintained in a plastic bag with the child's name on it, but the original box with the prescription label attached was missing.

1. In the infant classroom, two containers of infant cereal were not labeled with Child #9's name.
Evidence:
1. In the infant classroom, three bottles for Child #7 were not labeled with the child's name and date.
2. In the infant classroom, two bottles for Child #8 were not dated.
Evidence:
1. Child #2's record (Start date-10/31/2022) did not have documentation of a second emergency contact's name, address, and phone number.
