Inspector Notes

An unannounced complaint investigation was conducted on July 24, 2023 from 10:30am through 11:30am. Staff Interviews were conducted, records reviewed, and observations made in the Pre-K classroom in response to a complaint alleging rough handling. The preponderance of evidence does not support the allegations and the complaint has been deemed not valid. Any violations cited were found during the course of the investigation. Based on record review, each child did not have a physical examination by or under the direction of a physician within 30 days after the first date of attendance.
Evidence:
1. Child #1's record (Start date 11/01/2022) did not contain documentation of a physical examination under the direction of a physician.
Plan of Correction: The child is no longer enrolled at the center, but the management team will ensure that all children enrolled at the center will have a physical examination in their record.
