Inspector Notes
An unannounced renewal inspection was conducted on September 11, 2023 from 9:45am-12:30pm. At the time of entrance, 60 children were in care with 10 supervising staff members present. The inspector reviewed 5 children's records and five staff records. Child were observed during handwashing, circle time, coloring, bottle feeding, centers, and group activities. The Emergency Preparedness Plan, Evacuation Drill logs were reviewed and complete. Areas of non-compliance are identified on the violation notice. Repeat violations were found and are identified within this report. Violations
7Evidence:
1. Staff #3's record (Date of hire-08/18/2003) contained an expired criminal history record check. Documentation of a criminal history record check was dated 03/29/2018.
Evidence:
1. The center that was built before 2015 was not equipped with at least one carbon monoxide detector.
2. Staff #6 confirmed that there was not one carbon monoxide detector in the center.
Based on record review, staff members did not obtain and submit subsequent tuberculosis(TB) screenings.
Evidence:
1. Staff #1's record (Date of hire-03/29/2021) did not contain a subsequent TB screening. Documentation of the last Tb screening was dated 03/25/2021.
Based on observation, areas and equipment of the center, inside and outside, were not maintained in a clean, safe and operable condition.
Evidence:
1. In the Two's classroom, the peddle operated trashcan was not operational. Staff members had to open the lid of the trashcan with their bare hands.
2. In the Dramatic Play area of the Early Pre-school classroom, one knob of the kitchen faucet was missing and exposing a protruding sharp one inch screw.
3. In the Pre-school classroom, there was not a peddle on the peddle operated trashcan.
4. On the playground, there were two tricycles with broken plastic seats that exposed sharp edges that could entangle clothing and snag skin.
Based on observation, hazardous substances were not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. In the Toddler classroom, a bottle labeled bleach and water was sitting on a high diapering table of the classroom and not locked up.
2. In the bathroom of the Early Pre-school classroom, a container of diaper cream was located in a basket with diapers on a low shelf accessible to children.
Based on observation, cots were not identified for use by a specific child.

1. In the Pre-K classroom, the cots were not identified for use by a specific child.
Based on record review, children's records did not contain required information.
Evidence:
1.Child #1's record (Start date-11/18/2019) did not contain documentation of child updates and confirmation of up to date information in the child's record.
