Inspector Notes
An unannounced inspection was initiated on May 5, 2025 and completed on May 7, 2025 in response to a compliant received by the licensing office on May 5, 2025 relating to allegations of staffing and supervision. The inspector reviewed one child's records and five staff records on-site. This inspection included document review, tour of the facility, interviews and observations.
The preponderance of evidence gathered during the investigation supports the allegations; therefore, the compliant is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on May 19, 2025. A POC submitted after this date will not appear on the public website.
Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection and guidance. On 4/29/25, Staff #1 took 6 children over to classroom B, then exited the classroom, leaving Child #1 unsupervised for approximately three-to-four minutes before staff realized the child was not with the group.
Plan of Correction: Meeting with Staff #1 and written final. Discussion of Company / and licensing requirements, and the importance of health and safety for all children. discussion of the importance of using name-to-face to account all children when transitioning children. We will ensure all children are accounted for and safe while transitioning children.
Standard 8VAC20-780-340-F
Children under 10 years of age shall always be within actual sight and sound supervision of staff. On 4/29/25, one child was unsupervised for approximately three-four minutes.
Plan of Correction: Meeting with full staff, discussion of company / and licensing requirements and regulations. The importance of children safety, using name-to-face to account all children in ratio. We will ensure all children are accounted for and safe while transitioning children.
Standard 8VAC20-780-350-B-2
Repeat Violation: The center failed to follow the required ratio for children 16 months-2 years of one staff per five children. Staff #1 took six two-year old children to classroom#2 that had four children, ranging from 16-24 months and one staff member, Staff #6. Staff #6 was left alone with a group of eleven children, ages 16 months to 2years, for approximately 3-5 minutes.
Plan of Correction: Staff #4 will not transfer children to another classroom.
Staff #2 will not accept children from another class as it will put her out of ratio.
Staff #1 and staff #2 have completed 3 hour Health and Safety training.
We will ensure all staff are complying with classroom ratios.

Repeat Violation: The center failed to follow the required ratio for children ages 2 year olds of one staff per eight children.
On 4/30/25, Staff #4 supervised nine children from approximately 7:38am to 7:41am.
On 5/1/25, Staff #2 supervised 19 children from approximately 7:35am-8:00am.
On 05/02/25, Staff #2 supervised 24 children from approximately 7:36am - 8:06am.
On 05/06/25, Staff #4 supervised 14 children from approximately 7:35am to 8:00am.
Plan of Correction: Staff schedules have been adjusted to meet classroom ratios. We will ensure all classrooms are always in ratio compliance for the safety of children.

The center failed to follow the required ratio for children three year olds and up to school age eligible of one staff per ten children.
On 4/30/25, Staff #2 supervised 24 children from approximately 8:05am to 8:55am.
On 5/1/25, Staff #2 supervised 22 children from approximately 8:10am to 8:45am.
On 5/2/25, Staff #2 supervised 24 children from approximately 8:00am to 8:59am.
On 5/6/25, Staff #2 supervised 24 children from approximately 8:41am to 8:53am.
Plan of Correction: Staff schedules have been adjusted to ensure staff #2 classroom meets ratio.
We will ensure all classroom are always in ratio compliance for the safety of children.
Standard 8VAC20-780-40-E
The operational responsibilities of the licensee shall include ensuring that the center's activities, services and facilities are maintained in compliance with the center's own policies and procedures that are required by these standards. The Face-to-Name Transition sheet did not have complete documentation of the arrival and/or departure of children and staff in the classrooms on 4/29/25, 4/30/25, 5/1/25, 5/2/25 and 5/6/25.
Plan of Correction: Meeting with full staff, discussion of the importance of Name-to-face with instruction of the correct way to complete name-to-face. Teachers job aid of Name-to-face given to staff to support them if they need assistance.
