During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced inspection was initiated on January 29, 2026 and completed on January 29, 2026 in response to a complaint received by the licensing office on January 23, 2026 relating to allegations of neglect. The inspector reviewed 1 child?s record and 3 staff records on-site
on January 29, 2026. This inspection included document review, tour of the facility, interviews, and observations.
The preponderance of evidence gathered during the investigation does not support the
allegation; therefore, the complaint is determined to be not valid. Though not a part of the
original complaint, violations that were not reported were found and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation
cited on the violation notice. Specify how the violation will be or has been corrected. Submit your
POC within five (5) business days from today, which will be the close of business on February 4, 2026. A POC submitted after this date will not appear on the public website.

The center shall obtain documentation that each child has received the immunizations required before the child can attend the center.
